When both the neck and the shoulder hurt

The short answer

Add height before you change anything else. When the shoulder is involved, you are almost always a side sleeper on a pillow that is two inches shorter than your shoulder needs, so your head hangs into the gap, your neck bends toward the mattress, and your shoulder carries the load at an angle for eight hours. Why both hurt at once

One gap, two joints paying for it.

Lying on your side, your shoulder lifts your whole upper body off the mattress and leaves several inches of empty space under your head. What happens next decides whether you wake up sore in one place, two, or neither.

Measure it, do not guess

Have someone look at you from the front.

Lie on your side the way you sleep, with your head on the pillow you own now, and have someone stand at the foot of the bed and look straight at you. If your head is tilted down toward the mattress, the pillow is short. If your chin is being pushed up toward the ceiling, it is too tall. Level is the whole target and it takes about ten seconds to check.

Most side sleepers land between four and six inches once they get it right, and broad shoulders need the top of that or more. Nearly everyone we talk to about a sore shoulder has been sleeping shorter than that.

Then make the height stay

A pillow that gives an inch back has undone your work.

Setting the right height is only half of it. A soft fiber pillow can be perfect at bedtime and two inches shorter by the time your head has been pressing on it for four hours, which puts you straight back into the gap you were trying to fill.

Buckwheat locks and does not move. Dense organic cotton settles so slowly you measure it in years. Latex pushes back every time you shift. Those three hold the number you set, and holding it is the point.

Your knees are pulling on the same shoulder.

Side sleepers fix the head and leave the bottom half alone. Your top leg slides forward and down across the bottom one, which rotates your pelvis, twists the spine, and pulls through your ribs into the shoulder you are lying on.

A body pillow between the knees keeps your hips stacked so the twist never starts. It is the single most under-used fix for a sore shoulder, and it costs you nothing to try on the pillow you already own before you buy ours.

What is the best pillow for neck and shoulder pain?

Why does my shoulder hurt after sleeping on my side?

Should I sleep on the sore shoulder or the other one?

What loft do I need if my shoulders are broad?

Can a pillow cause shoulder pain?

Does a body pillow help shoulder pain?

More than most people expect, and by an indirect route. It stops your top leg dragging your pelvis into rotation, and that rotation travels up the spine and into the shoulder you are lying on. It also gives your top arm somewhere to rest that is not across your own body.

How long before my shoulder feels better?

Give it a week or two once the height is right. Sleep three nights at any given height before you change it again, and give the pillow a few weeks overall. The sixty nights exist so nobody has to judge this early.

My arm goes numb at night. Is that the pillow?

It can be pressure through a poorly held shoulder, and it can be several things that are not the pillow at all. Numbness that persists after you get up, or travels below the elbow, is worth a clinician's opinion rather than another pillow purchase.
